Do you need a copy of an award nomination form?

• Click here if you have MS Word 2003 (or newer) or want the "PDF" format for any of the awards in the following group:
  • Outstanding Patroller (Alpine, Nordic, Instructor, Administrative),
  • Distinquished Service,
  • Meritorious Service,
  • Merit Stars/National Certificate of Appreciation
    (use the Merit Star form also for Central Division Critical Care Certificate),
  • National or Leadership Commendation Appointment,
  • Outstanding Alpine or Nordic Patrol,
  • Patroller's Cross,
  • Minnie Dole Award - Nomination

• After logging in:
  1. click on Member Resources,
  2. choose My Bookshelf from the Member Links drop-down menu,
  3. choose Downloads from the Reference & Resource Materials section,
  4. select the nomination form, and open or save the file.
• The file you have downloaded is an MS Word document, requiring MS Word 2003 or newer.
• If you don't have MS Word 2003 (or newer), choose the "PDF" (Adobe Acrobat Reader) format, print the form via the Adobe Acrobat Reader, and complete the form by hand.)

A message from the Western Region Awards Advisor:

The National Ski Patrol recognizes excellence and dedication at all levels through public expressions of appreciation in the form of awards. The awards are sincere expressions of appreciation for accomplishments in the service of the NSPS and the skiing public.

Any time is the right time to be thinking about awards for your fellow patrollers who deserve recognition and commendations for their service. I request that you send me one copy for proofing so that I have time to contact you if additional information is needed before the next Awards Committee meeting. The additional copies that are needed can be hand-carried to the meeting by a member of the committee.
